Request for Access to Hospital Medical Record Information

In accordance with the Health Records Act 2001 a patient can request to access information from the hospital medical record.

Once the application form has been received by the hospital, the costs involved will be provided and the process to search for the information will commence.

In accordance with the Health Records Act 2001 and the Privacy Act 2014, it may take 45 days to provide the requested information.

Fees & Charges:

Under the Health Records Regulations 2012 there are regulated costs involved in providing this information and an invoice of the charges will be sent once a request is granted.

Standard application fee: $51.90
Express registered post: From $8.00 as per weight
Assess and collate medical record: $36.10
Photocopying: 0.20c per page
Retrieval from secondary storage (if applicable): $17.00

Note: Albert Road Clinic hospital medical records include documents such as inpatient clinical notes and discharge summaries.
If you require a copy of your treating psychiatrist’s medical records you need to request this directly to the psychiatrist. The hospital does not have access to these records.

Should you wish to make an application to access information from your medical record please complete the application form and post it to Albert Road Clinic together with a photocopy of photo identification (e.g. your driver’s licence or passport) to:
