Patients of the Clinic can be referred to the Outreach Service, and be assigned to an Outreach clinician who visits the patient in their home or residence.
The service provides short-term support and problem-solving to help people during and after acute episodes of illness. Outreach also provides assistance for longer-term illnesses through motivational support, and lapse/relapse management. Outreach is often employed in collaboration with other treatment modalities (e.g. day programs) to offer the treating doctor and the patient a comprehensive and flexible plan of treatment.
The primary goal of the Outreach Service is to facilitate maximum recovery, via ongoing treatment, support and individual rehabilitation, which is provided in the home environment. This can help patients in their transition back home from hospital. It may also reduce the need for inpatient care.
Each Outreach clinician has specialist clinical training in working with adolescents, and adults including the elderly.
The Outreach Service operates Monday to Friday from 9.00am to 5.00pm (travel distance criteria apply).
